First, it's important to target who are the right people to speak to at the hospital.
That would be the discharge planning team at the hospital. The discharge personnel are responsible for the important step of managing the discharge of a patient from the hospital and they are responsible and accountable to the patient for a safe discharge.
What is safe? If the discharge scenario requires care in the home, it would mean that the patient has someone, either a family member, friend or caregiver that is available to be in the home when the patient is discharged, and the caregiver can physically care for the patient based on their needs.
When is the right time in your home care journey to approach the discharge planners in a hospital?
I’d recommend not approaching a hospital until you have some experience and references in the market. Understand the discharge planers have already been approached by many home care agencies and their first question will be what makes you different, how long have you been doing it and if we give you a referral, how fast can you staff it?
If you are green they probably will not pay attention to you. You should come in ready with an objective to make them feel they can count on you and trust you for good care.
How can you get their attention to help you rise to the top of their referral list? Speaking from experience, we took the approach to partner with other key organizations and providers in a hosted “lunch and learn” program at least once a quarter. We would partner with home health and hospice providers, Geriatric Care Managers (Aging Life Care Professionals), and The Dementia Alliance of NC, to name a few.
We would pick a topic or concern of the discharge planners and bring in lunch and talk while they ate. It was highly successful, and it helped us rise to the top of their list for years.
